4. Analytical fee of Rs. 4657.50/= (subject to change) in favor of the Director I.T.I. Colombo.

Submissions to be made by the Manufacturer of the Coil/mat /Repellent

(a) Name and address of Manufacturer of Mosquito Coil/Mat/Repellent
(b) Name and address of the Manufacturer of active ingredient/technical material
(c) A sample of two boxes of the product
(d) Analytical Quality Certificate of the test sample
(e) Specific detailed composition of the Coil/Mat/Repellent
